You may be eligible for up to two meals a day delivered weekly to your home from one of our partner kitchens.

Let Colorado Food Cluster serve you! If you think you may qualify, you should contact your case manager or case management agency (CMA) to confirm eligibility and request Colorado Food Cluster as your Home Delivered Meal Provider.

Learn more about qualifying. You may qualify for meals if you are an adult who receives health coverage through Health First Colorado and are:

  • Returning home from a hospital stay, or

  • Transitioning to community living, and enrolled in an adult waiver program, or

  • Experiencing a qualifying change in life circumstance.
